Doomsday Defense Dojo Participant Waiver & Release of Liability

I understand the contagious nature of COVID-19 and voluntarily assume the risk of exposure by attending classes or using facilities. I agree not to attend if I experience symptoms, have tested positive in the last 10 days, or have had close contact with a confirmed case. I acknowledge that the dojo follows safety protocols but cannot guarantee zero risk.

I, the undersigned, acknowledge and fully understand that participation in martial arts, boxing, Muay Thai, Brazilian Jiu-Jitsu (Gi & No-Gi), grappling, self-defense, and fitness activities at Doomsday Defense Dojo, or any activities conducted by or with John "Doomsday" Howard or instructors contracted by Doomsday Defense Dojo, involves physical contact and exertion which may carry a risk of serious injury, illness, disability, or even death. I voluntarily assume all risks, both known and unknown, and accept full responsibility for my participation.

I certify that I am in good physical condition and have no known medical or mental conditions that would prevent me from safely participating in training. I agree to inform the staff of any medical conditions prior to engaging in any activities.

In consideration for being allowed to participate, I hereby release and hold harmless Doomsday Defense Dojo, John Howard, its staff, instructors, affiliates, guests, and property owners from any and all claims, demands, damages, actions, or causes of action arising from or related to any loss, injury, or illness I may sustain while participating. This waiver applies to any incidents occurring on or off-site, including seminars, competitions, demonstrations, and travel.

I understand that classes may be recorded or photographed for promotional or educational purposes. I grant permission for Doomsday Defense Dojo to use my image and likeness unless I notify them in writing.

I agree to abide by all safety rules, guidelines, and instructions given by the instructors or staff. Failure to do so may result in dismissal from classes without refund.

I understand that private sessions and classes must be canceled at least 24 hours in advance to avoid charges. No-shows or late cancellations may result in forfeited sessions. Merchandise purchases are final sale unless otherwise noted.
